Please read info below before submitting form and payment. Deadlines for the early price to be honored must be paid before each pageant deadline listed.

At the pageant you will need to see us during competition to give us your child's competition number.
If you do not see a pageant listed on the form that is on our 2009 schedule, that means we are not yet accepting early payment.
DVD's in these packages are sold by the age group only, we reserve the right to combine age groups but not necessarily into supreme groups.
The price below of $90 is only good before the pageant registration day. Price is not honored at the pageant or after the pageant. Do not use this form if today's date falls after the deadline dates for the listed pageants.

	Early order price is below. A discount will be offered at the pageant but not in this amount.
	$90 Photo CD/DVD Supreme PKG (Full price $120) This includes all onstage events for regular competition and the DVD of your child's age group. (DVD's are sold by the individual age groups, however we reserve the right to combine age groups at each pageant based upon the number of contestants and events of competition but not always combined into supreme groups) Any talent or crowning DVD's must be purchased separately from your competition pkg. These are NOT included in your pkg.
